A practice gap means the student understands the idea but hasn't done it enough times for it to feel automatic. The fix is straightforward: more well-designed practice. A concept gap is different — it means something underneath the current topic never fully clicked, and that shaky foundation is quietly sabotaging everything built on top of it.

A student who never truly understood how fractions relate to division will struggle with ratios, then with slope, then with rates of change in Calculus 12 at UBC or SFU — and every teacher along the way will think it's a "new" problem. v² = v₀² + 2·a·Δx
...has one brittle fact. A student who understands it knows it comes from combining velocity and displacement — that it's the tool you reach for when you have distance but no time. That student can adapt when the problem is phrased sideways. Same equation, completely different security.
Or consider a genetics cross in Biology 12. Memorising "9:3:3:1" is fragile. Understanding why a dihybrid cross of two heterozygotes produces:
9 : 3 : 3 : 1 (from a 3:1 ratio multiplied by a 3:1 ratio)
...means you can rebuild the ratio from scratch, and adjust it when a question changes the parental genotypes. The memoriser panics; the understander reasons.
